The NGA Center’s disease management work examines how such programs can impact public health insurance programs like state employee benefits and Medicaid.

Disease management programs can vary, but typical programs provide health coaching to assess barriers to change and provide motivation. Other components include waiving or reducing costs of office visits, medications, and clinical work to prevent catastrophic outcomes and to improve quality of life. In other cases, incentives are offered to providers to coordinate care among the health care team so that the individual does not receive duplicative orders and to ensure an individual with various chronic diseases receives the right attention.

The NGA Center’s health division analysts and experts are available to assist gubernatorial offices and states with technical assistance on disease management.
